Job Responsibilities: Assess patients' physical and mental abilities to determine rehabilitation needs, focusing on activities of daily living (ADLs) and functional mobility within a Skilled Nursing Facility (SNF) setting
Develop and implement individualized treatment plans designed to help patients regain or improve their independence in daily activities
Provide therapeutic interventions to enhance fine motor skills, coordination, strength, and mobility, using a variety of techniques and equipment
Educate patients and their families on adaptive techniques and the use of assistive devices to enhance safety and independence at home
Collaborate with a multidisciplinary healthcare team, including physical therapists, speech therapists, and nursing staff, to ensure comprehensive patient care
Monitor and document patient progress, adjusting treatment plans as necessary to achieve optimal outcomes
Assist in the discharge planning process, providing recommendations for continued therapy, home modifications, and support services
Maintain a clean, organized, and safe therapy environment, adhering to infection control and safety protocols
Participate in regular team meetings and contribute to the development of care plans and goal setting for patients
Engage in professional development activities to stay current with best practices, regulatory requirements, and advances in occupational therapy
Job Requirements: Strong understanding of occupational therapy principles, especially in the context of a Skilled Nursing Facility (SNF) environment
Excellent communication and interpersonal skills for interacting with patients, families, and a multidisciplinary team
Proficiency in creating and implementing individualized treatment plans based on patient needs
Active [STATE] Occupational Therapist (OT) License in good standing
A minimum of 2 years of experience as an Occupational Therapist or in a similar role
Certification in Basic Life Support (BLS)
Certification in Advanced Cardiovascular Life Support (ACLS)
Experience with EPIC or similar charting systems
