**Job Title:** Home Health Therapist

**Shift:** Day Shift, Monday to Friday, 8:00 AM - 4:30 PM
**Guaranteed Hours:** 40 hours per week
**Experience:** Minimum 1 year
**License:** State License required; pending state license accepted (Missouri license or compact license preferred)
**Certifications:** Basic Life Support (BLS); EPIC charting experience required
**Must-Have:**
- Case management skills
- OASIS understanding
- Independent with strong time management and organizational skills
- Ability to set patient-centered goals
- Experience supervising Certified Occupational Therapy Assistants (COTA)
- Team player with professionalism and excellent communication skills
- Flexibility and positive attitude
- Commitment to providing excellent quality care

- Manage a daily caseload of 5-6 units per tech.
- Maintain accurate and timely patient records using EPIC.
- Collaborate effectively with healthcare team members to achieve patient goals.
- Provide guidance and supervision to COTAs.
- Develop and implement individualized care plans based on patient needs.
- Communicate effectively with patients and their families to ensure understanding and satisfaction with care.
- Demonstrate flexibility and adaptability in a dynamic work environment.
