Pay Rate: $1549.76 - $1785.59

8:30 P-8A / 13 Weeks
Type of equipment – PyxisES, DoseEdge, KitCheck, Vigilanz, OnBase, Medex
EMR - Meditech
Must have: candidates without these skills will not be considered for the role.
• Vancomycin, aminoglycoside, warfarin, heparin dosing
• Medication order verifications
• IV product verifications
• Renal Dose adjustments, IV to PO Conversions
REQUIRED CERTIFICATIONS:
• Texas Pharmacist License
• IV Certificate
Preferred or nice to have: candidates with these skills will be considered first.
• Critical Care Experience
• NICU Experience
• Pharmacy Residency
• Board Certification
List typical procedures performed on unit(s):
• Medication Order Verification
• Medication Dosing Consults (Vancomycin, Aminoglycosides, Warfarin, enoxaparin, acetylcysteine)
• Antimicrobial stewardship, opioid stewardship
• Working through automatic protocols such as IV to PO conversations, Renal Dosing
