Who will be the DOO approving the contractors time each week? Tracy Walker What is the iCIMS req # corresponding to this requisition? 2024-29865 How long is the contract assignment? 13 weeks is the norm: 13 weeks Schedule of assignment hours? M – F 8am – 5pm? Yes Will there be any weekend rotation? If so, how often? Yes, one weekend per month Will there be any on call? If so, how much? One week per month, includes the above weekend What region are you in? Home health or Hospice? Home Health What territory this contractor will be asked to cover? Southern Indianapolis, Greenwood, Shelbyville, Franklin What’s creating the request? Full time clinician decreased to PRN status Which location will this person report to for their assignment- add info below Full address: 10475 Crosspoint Blvd, Suite 105, Indianapolis, IN 46256 OU# (location code) 1874 About how many miles per week will the contractor travel? 200 approximately What are the weekly units of productivity expectations? Ex: 25/30 ? Minimum of 30 points per week What EMR systems will the consultant be using: HCHB/Pointcare Do you want the candidate to have experience in have home health, hospice or both ?
