Job Responsibilities: Provide in-home physical therapy services to patients recovering from illness, surgery, or injury, helping them regain mobility and independence in the comfort of their own home
Conduct comprehensive assessments of patients' physical abilities, mobility, strength, balance, and functional limitations to develop personalized treatment plans
Design and implement therapeutic interventions, including exercises, gait training, and mobility techniques, to improve patient strength, coordination, and range of motion
Educate patients and their families on home exercise programs, techniques for improving mobility, and safety measures to prevent falls or injuries in the home
Collaborate with physicians, nurses, and other members of the healthcare team to ensure a coordinated and comprehensive approach to patient care
Regularly assess and monitor patient progress, adjusting treatment plans as needed to ensure optimal recovery and achievement of therapy goals
Document patient evaluations, treatment plans, and progress notes in the electronic health record (EHR), ensuring compliance with home health regulations and standards
Provide recommendations for adaptive equipment, such as walkers or grab bars, to support patient mobility and safety at home
Participate in discharge planning, ensuring patients receive continued support or outpatient therapy services as needed after home health care
Stay up-to-date with advancements in physical therapy, home health best practices, and rehabilitation through continuing education and professional development
Job Requirements: Strong understanding of physical therapy principles, rehabilitation techniques, and home health care protocols
Excellent communication and interpersonal skills for working with patients, families, and healthcare teams in a home health environment
Proficiency in conducting assessments, creating individualized therapy plans, and implementing physical therapy interventions
Active [STATE] Physical Therapist (PT) License in good standing
A minimum of 2 years of experience as a Physical Therapist (PT), preferably in a home health or rehabilitation setting
Completion of an accredited physical therapy program (Master's or Doctoral degree)
Certification in Basic Life Support (BLS)
Experience with home health documentation systems (e.g., EPIC, Homecare Homebase, etc.)
