A skilled nursing facility in Lock Haven, PA is looking for a dedicated Medical Physical Therapist PT to join their team for a 13 - week contract with the possibility to extend. Responsibilities of a Skilled Nursing Facility SNF Physical Therapist PT: * Develop effective treatment plans and obtains approval for services from referring physician * Evaluate residents to obtain data necessary for treatment planning and develop a treatment plan that includes a target list, short-term and long-term goals * Participate in service training program for other staff in the facility * Communicate with supervisor and other health team members regarding patient progress, problems and plans * Possesses and conveys high degree of credibility and integrity * 13 week assignment * 36 hours guaranteed * Submits all billing logs and necessary information on a timely basis Qualifications of a Skilled Nursing Facility SNF Physical Therapist PT: * Doctorate in Physical Therapy * Must have an active Physical Therapist PT license * Current BLS / CPR Certification Pioneer Healthcare Services is a San Diego-based staffing and travel recruitment firm that specializes in finding exceptional nursing and therapy positions for its clients across the US.
