**Job Description:**

- Perform physical therapy evaluations and treatments for patients in an outpatient setting.
- Collaborate with a multidisciplinary team to establish and implement effective therapy plans.
- Educate patients and families about treatment plans and exercises to continue at home.
- Maintain accurate and timely documentation of patient care and progress.
- Participate in departmental meetings and contribute to continuous quality improvement initiatives.
- Utilize available resources to deliver cost-effective care in a rehabilitative environment.
- Free shared housing available; no guests or pets permitted.
- Reliable transportation is required.
