**Job Description:**
- Evaluate patient/resident status and needs.
- Plan treatment and establish realistic and measurable short-term and long-term goals in conjunction with the referring provider, patient/resident, family/caregivers, and other team members.
- Provide and evaluate the effectiveness of therapy treatments and plans according to department policies and procedures.
- Educate patients/residents, family/caregivers, and other team members to enhance the attainment of established goals.
- Deliver care/treatment in a manner appropriate for the patient's/resident's age, physical ability, and intellectual development.
- Complete accurate, legible, timely, and effective documentation according to department policies and procedures.
- Communicate clearly and concisely with team members, referring providers, and insurance/case managers regarding patient/resident status, progress, needs, and discharge planning.
- Delegate appropriate patient/resident treatments to therapy assistant staff and provide supervision and direction to therapy assistants.
- Complete reevaluations in cooperation with therapy assistants as indicated and according to department policy and procedure.
- Complete daily billing accurately and schedule patients efficiently in cooperation with the receptionist.
- Participate in department and hospital-wide educational opportunities for continuing professional development/education.
- Provide one department in-service if attending a continuing education course that is reimbursed.
- Participate in the Rehab Department student program, serving as a clinical instructor, mentor, and role model as available and appropriate.
- Participate in one Rehab Department or project, PI initiative, or committee.
- Adhere to all safety policies and procedures, reporting incidents, falls, adverse drug reactions, and equipment issues according to hospital policies.
- Participate in the development of and adhere to the department Mission, Values, and Ground rules, embracing cultural changes and implementation.
