A skilled nursing facility in Porterville, CA is looking for a dedicated Medical Physical Therapist PT to join their team for a 13 - week contract with the possibility to extend.�Physical Therapists PTs who work for Pioneer Healthcare Services are dedicated professionals who interact amongst diverse populations and are enthusiastic about the difference they make in their patients' lives through assessment and treatment.�Experienced and new grad Physical Therapists PT are encouraged to apply
Responsibilities of a Skilled Nursing Facility SNF Physical Therapist PT:
• Develop effective treatment plans and obtains approval for services from referring physician
• Evaluate residents to obtain data necessary for treatment planning and develop a treatment plan that includes a target list, short-term and long-term goals
• Participate in service training program for other staff in the facility
• Communicate with supervisor and other health team members regarding patient progress, problems and plans
• Possesses and conveys high degree of credibility and integrity
• Submits all billing logs and necessary information on a timely basis
Qualifications of a Skilled Nursing Facility SNF Physical Therapist PT:
• Doctorate in Physical Therapy
• Must have an active Physical Therapist PT license
• Current BLS / CPR Certification
