**Job Title:** Physical Therapist
**Job Specialty:** Rehabilitation Therapy
**Job Duration:** 26 weeks
**Shift:** Days: 8:30 am - 5:00 pm (Flexible based on patient availability); One weekend day
**Guaranteed Hours:** 40 hours per week
**Experience:** Minimum 1 year of Home Health experience
**License:** Required
**Certifications:** N/A
**Must-Have:** Proficiency in Homecare Homebase EMR
**Job Description:**
- Focus on restoring function to prevent or minimize disability following disease or injury.
- Assist clients in reaching maximum performance and function within their capabilities.
- Evaluate and treat patients using medically prescribed physical therapy programs.
- May supervise Physical Therapy Assistants.
- Conduct initial admission/evaluation visits.
- Notify assigned Clinical Team Manager of OASIS scores within established timelines.
- Document observations, clinical findings, problems, skilled interventions/treatments, goals, and discharge plans.
- Perform duties consistent with skills and training, aligned with the organization's goals and mission.
- Work within territories including Milpitas, Saratoga, Monte Sereno, Los Gatos, Campbell, San Jose, Morgan Hill, San Martin, Gilroy, Coyote Point, Sunnyvale, and neighboring cities as needed.
