**Experience:** Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience

**License:** Must have a current license in hand

**Certifications:** CPR, BLS

**Must-Have:**
- Proficiency in HomeCare HomeBase (HCHB) used within the last 6 months or completion of a retraining course
- Proficiency in OASIS for RNs, PTs, and OTs
- Reliable transportation, a valid driver's license, and agency-required liability insurance
- Comfort with traveling up to 75 miles from home clinic
- Ability to complete documentation within 24 hours of patient appointment

**Job Description:**
- Provide, coordinate, and direct the provision of home nursing care according to physicians' orders, based on agency policies and procedures
- Supervising and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses
- Conduct home visits and participate in periodic office meetings
- Manage an average of 5-8 patients per day
- Handle various types of cases including Start of Care, Wound Care, and Follow-up Visits
- Participate in on-call rotations as required
- Meet weekly productivity expectations of 25-30 points
- Engage in floating to nearby clinics if necessary to maintain financial stability

Applicants should be prepared to travel between patient homes daily and periodically visit the home clinic for meetings
Effective communication and documentation skills are essential, along with the ability to manage a variable schedule based on patient needs.
