**Job Title:** Physical Therapist Assistant (PTA) Home Health
**Job Specialty:** Home Health Therapy/Rehabilitation
**Job Duration:** 13 weeks
**Shift:** Day, 5x8-Hour (08:00 - 16:30)
**Guaranteed Hours:** Guaranteed work week
**Experience:** Minimum 1 year of skilled home health experience
**License:** Pending license accepted
**Certifications:** BCLS
**Must-Have:**
- Own transportation
- Ability to visit up to 6 patients per day
- References covering 1 year of employment within the last 3 years
**Job Description:**
- Provide skilled rehabilitation services to patients in a home health setting
- Complete patient documentation using EPIC charting system
- Participate in a full orientation program of 40 hours
- Adhere to a 1:1 patient-to-therapist ratio
- Work one weekend shift per contract
- Floating between facilities within the state is expected
- Opportunity to work with low submittals and consideration for local candidates within a 50-mile radius
- Participation in non-billable modules, completed pre-start and annually, is required
- Option for medical/religious exemptions or declinations for COVID-19 vaccine requirements
This opportunity requires professionals who are organized, patient-focused, and possess exceptional communication skills
Ideal candidates will have Oasis experience, although it is not mandatory
