Weekly Pay Range - $1039 To $1154

**Certifications:** Certification as a Certified Occupational Therapy Assistant (COTA)

**Must-Have:**
- Complete prescreen form, resume, and license verification must be attached upon submission
- COVID test to be taken 72 hours prior to the start date with results issued 48 hours beforehand

**Job Description:**

- Ensure the prescreen form is completed and attached upon candidate submission
- Submit resume and license verification with application
- Collect and submit three professional references
- Conduct comprehensive background checks, including:
- County criminal check (7 years)
- Federal criminal check (7 years or more)
- Multi-state criminal search (7 years or more)
- Nationwide sex offender search (7 years or more)
- HHS/OIG search
- GSA search
- Social Security Number Trace
- SAM search
- Washington State background check with fingerprints (facility-run)
- Required health screens:
- 2-step PPD or Quantiferon, or proof of positive PPD and chest X-ray
