**Job Description:**
- Ensure prescreen is completed and attached upon submitting the application
Candidates without this will be declined.
- Perform duties as directed to provide physical therapy assistance under the supervision of a licensed physical therapist.
- Participate in the rehabilitation plan by following therapeutic regimens designed for patients.
- Maintain patient records and document the patient progress under guidance.
- Conduct a county criminal check covering 7 years for all counties lived and worked in, along with federal and multi-state searches.
- Conduct a nationwide sex offender search covering the past 7 years or more.
- Complete HHS/OIG, GSA, SAM searches, and Social Security Number Trace.
- Provide proof of 1 Step PPD or Quantiferon or demonstrate proof of positive PPD and Chest X-Ray.
