Job Title: Registered Nurse (RN) - Long Term Care

Job Summary: As a Registered Nurse (RN) in Long Term Care, you will be responsible for providing comprehensive, patient-centered care to residents in a long-term care facility. You will work closely with other healthcare professionals to ensure the highest quality of care and improve the overall well-being of residents. Your role will involve administering medications, managing care plans, and offering emotional support to both residents and their families.

Key Responsibilities:

• Patient Care:

  • Assess and monitor patients’ health conditions and vital signs.
  • Administer medications and treatments as prescribed.
  • Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
  • Perform wound care, catheter management, and other specialized nursing tasks.

• Care Coordination:

  • Coordinate with interdisciplinary teams to ensure comprehensive care.
  • Communicate effectively with residents, families, and healthcare providers about care plans, progress, and changes in health status.
  • Document all aspects of patient care, including assessments, interventions, and outcomes, in electronic health records.

• Emergency Response:

  • Respond to medical emergencies promptly and efficiently.
  • Provide first aid and emergency care as needed.

• Resident Support:

  • Offer emotional support and education to residents and their families regarding health conditions, treatment options, and care routines.
  • Promote a comfortable and safe environment for residents.

• Compliance and Standards:

  • Adhere to facility policies, state regulations, and federal guidelines.
  • Ensure all care provided is compliant with safety and quality standards.

• Education and Training:

  • Mentor and supervise nursing assistants and other junior staff members.
  • Participate in continuing education and professional development activities.

Qualifications:

• Education and Licensure:

  • Associate's Degree in Nursing (ADN) or Bachelor’s Degree in Nursing (BSN) from an accredited institution.
  • Current and valid Registered Nurse (RN) license in [State].
  • Basic Life Support (BLS) certification required; Advanced Cardiovascular Life Support (ACLS) certification may be preferred or required depending on facility.

• Experience:

  • Previous experience in long-term care or a related field preferred.
  • Demonstrated ability to manage complex care situations and make critical decisions.

  • Strong clinical assessment and problem-solving skills.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively in a team environment.

Physical Requirements:

• Ability to lift and move patients as needed.
• Capability to stand, walk, and bend for extended periods.
