- Social Worker/Discharge Planner - 40 hrs Days
- Shifts: 8am – 4:30pmDay and rotating weekends
- Will accept RN Discharge Planner or Licensed Social Worker
- Social worker must be eligible for LMSW/LGSW/LCSW - they must have taken and passed the exam already
- Patient Diagnosis: Geriatrics, ICU, Heart and Vascular, General Surgery, General Medicine, MICU
- Holidays – as needed
- Weekends – Every 5-6 weekends must work one shift
- Cerner is required
- Uniform: Business Casual-Associate’s appearance should be clean, neat and professional. NO SCRUBS
- Does not accept 1st timers
Job Details

- Work Setting skills Required: Cerner, Acute Hospital, Insurance Company, Long Term Acute Care/Rehab/Skilled Nursing
- Case Management/Utilization Review skills required: Admission Criteria, Benefits Eligibility, Care coordination, Discharge Planning, Needs Assessment/Order DME, Plan of Care, Pre-Cert Review, Prior Authorizations, Worker’s comp case management
- Regulatory Skills Required: CMS: Centers for Medicare and Medicaid Services, Department of Health, HIPAA (The Health Insurance Portability and Accountability Act), The Joint Commission/ Core Measure/National Safety Goals
- Any Special Requirements: Must have experience with discharge planning, obtaining facility choices, sending referrals, issuing IMM and other Medicare letters
