In need of a LCSW with discharge experience required *** This person will need to be independent problem solver, clinical social worker would be preferred, and must have complex discharge experience. Must work well with the formal discharge planner on site and providing post care planning with the patient.Start date: ASAPShift: 5x8HR shifts (M-F 08:00-16:30)Length: 13 or 26 week options with possible extensionYears of experience REQ: 1 year, will accept new grads with clinical social work experienceFirst-timers accepted: Yes, willing to review with the right experienceWeekend REQ: possiblyCerts REQ: MSW and BLS, LCSW preferredWill this traveler need to float between like units: yes, floating as needed between discharge locations.Locals accepted: Not at this time. Must work 2 of the 3 winter holidays.Unit notes: This is a new program in the state of WA. This position will be part of the complex discharge pilot / enhanced care management.
