**Job Description:**
- Ensure all prescreening documents are completed and submitted with the application.
- Attach a current resume and proof of license verification for consideration.
- Candidates may be declined if required documents are not attached.
- Conduct therapy sessions focusing on speech rehabilitation.
- Provide treatment plans tailored to individual patient needs.
- Maintain documentation of patient progress and treatment plans.
- Collaborate with a multidisciplinary team for comprehensive care.
- Ensure compliance with all state and federal regulations regarding patient care.
- Conduct comprehensive patient assessments and screenings.
- Review the credentialing checklist, which includes:
- Resume and three references
- County criminal check for the past seven years
- Federal criminal check for the past seven years or more
- Multi-state criminal search for the past seven years or more
- Nationwide Sex Offender Search for the past seven years or more
- HHS/OIG Search and GSA Search
- Social Security Number Trace
- 2 Step PPD or Quantiferon or proof of positive PPD and chest X-ray
- SAM Search
- Bilingual capability and holiday coverage are not required for this position.
