**Job Description:**
- Provide specialized speech and language therapy to clients in their homes.
- Assess, diagnose, and treat speech, language, and communication disorders.
- Collaborate with other healthcare professionals to create comprehensive individual care plans.
- Monitor and document patient progress and adjust treatment plans as necessary.
- Educate patients and their families on communication strategies and techniques.
- Travel within assigned territories, Orange and Sienna, as outlined on the territory map.
- Maintain up-to-date patient records in compliance with regulatory standards.
- Demonstrate flexibility and adaptability to meet the needs of the home health environment.
