Treatment Nurse - Licensed Vocational Nurse (LVN) Requirements
- Active Licensed Vocational Nurse in California
- Fully vaccinated against COVID-19 including Booster shots
- Knowledge of long-term care patient needs
- Demonstrated ability to deal with fast-paced environment
Primary Treatment Nurse (LVN) Responsibilities
- Complies with the facility’s attendance policy requiring regular, reliable, and punctual attendance
- Participates in the initial admission evaluation of residents. Performs an evaluation using techniques including observation and inspection.
- Identify, manage, and treat specific skin disorders and primary and secondary lesions, such as skin abrasions, foot problems such as corns and callouses, decubitus ulcers, bacterial, parasitic and viral skin infections, scaling papular diseases, and benign tumors.
- Documents any pressure sores or other problems accurately with proper size, stage, site, length, width, depth, presence of drainage, discoloration, etc.
- Photographs pressure sores, extensive skin problems initially when identified and as indicated to document improvement or deterioration, and upon admission.
- Performs a weekly body check on all residents and submits a complete copy to the DON. Immediately calls physicians for appropriate treatment order on skin problems when identified and as needed.
- Maintains a pressure sore profile for every pressure ulcer, updated weekly and needed to reflect accurate measurement and progress, and ensures residents receive appropriate prophylaxis and treatment.
- Ensures that pain medication is provided prior to any resident’s treatments.
- Confers with the Care Planning Team in the development of the care plan.
- Initiates requests for consultation or referrals as needed.
- Responds to requests from the resident, physician, or nursing staff.
- Monitors and treats skin conditions per physician’s orders.
- Assists with dining program supervision as needed.
- Gives direct physical and psychological nursing care in emergencies, as well as in the presence of illness or disability in order to maintain life, provide comfort, reduce stress, and enhance the resident's ability to cope.
- Provides teaching to resident and family members of prevention and treatment of skin conditions.
- Maintains the confidentiality of all resident care information
- Ensures that all nursing care is provided in privacy and that nursing service personnel knock before entering the resident's room.
- Provides treatment for skin disorders within established wound care protocol.
- Monitors the use of preventive measures such as appropriate bed/mattress with proper turning and positioning.
- Answers signal lights, bells, or intercom system promptly to determine patients’ needs.
- Monitors condition changes and properly documents and follows-up as necessary.
- Maintains a skin rash log and updates as necessary.
- Monitors and assesses the surgical site (staples, sutures, drainage).
- Monitors and assesses the in-dwelling catheter (tubing, sediment, blood).
- Monitors and assesses resident chest tube, Jackson Pratt, tracheotomy, colostomy, etc.
- Reports skin breakdown (new and decline) to physician and family in a timely manner.
- Performs other duties or functions as assigned by the DON, Administrator, or RN.
PAY SCALE: $34.00 to $38.00
