Triage Care Advisor RN Full TimeThe Triage Care Advisor Registered Nurse supports BMG ambulatory primary care and specialty practices through triaging calls and coordinating care as needed. The Triage Care Advisor Registered Nurse serves in an expanded health care role to collaborate with the primary care practices, specialists, and other members of the health care team, and patients/families to ensure the delivery of quality, efficient, and cost-effective health care services. The Triage Care Advisor Registered Nurse follows evidence-based clinical guidelines, preventive guidelines, and protocols in care delivery to the patient. The Triage Care Advisor Registered Nurse coordinates care across acute and other ambulatory settings and helps patients/families understand heath care options.
Additional Job Descriptions:
- Bachelor degree (BSN or BS) preferred and two to five years of experience in nursing with adult and pediatric patients in primary care/ambulatory care, home health agency, skilled nursing, or hospital based medical-surgical setting.
- Experience in ambulatory care is desirable experience as well as acute care or post-acute care setting.
- Licensed Registered Nurse in good standing for the State of Michigan
Essential Duties and Responsibilities:
- Ability to utilize word processing, spreadsheet, presentation programs, databases, and other software relevant to the job.
- Demonstrates customer focused interpersonal skills to interact in an effective manner with practitioners, patients with diverse opinions, values, and religious and cultural ideals.
- Demonstrates ability to work autonomously and be directly accountable for practices.

- Demonstrate ability to set appropriate boundaries with patients and families.

- Ability to affect change, works as a productive and effective team member, and adapt to changing needs/priorities.
- Work which produces high levels of mental/visual fatigue, e.g., interactive and repetitive or small detailed work requiring alertness and concentration for sustained periods of time.
- Requires excellent communication skills and positive customer relations. Regularly communicates both orally by telephone, and in writing with patients. Also communication with physicians, and other healthcare professionals to exchange information pertinent to their information provided. Demonstrates effective problem-solving and critical thinking in care for patients.
- Knowledge of chronic conditions, evidence-based guidelines, prevention, wellness, health risk assessment, and patient education.
- Connects patients with Acute Care Managers and other resources as needed.
- Critical thinking skills and ability to analyze complex data sets. Ability to manage complex clinical issues utilizing assessment skills, resources, and protocols.
- Excellent assessment and triage skills. Ability to implement evidence-based interventions and protocols for chronic conditions.
- Flexibility to adapt to the needs of the department
- Knowledge of health concerns or changes in current health status and how they may affect patients in all age ranges from neonate through geriatrics for various medical specialties.
