Minimum Qualifications:
+ Bachelor's degree in any field.
+ 1-2 years of experience with care coordination, patient/community outreach, and/or scheduling.
+ 1-2 years of experience in a clinical setting.
+ Substitution: A combination of education and related technical/paraprofessional experience may be substituted for the bachelor's degree on a year-for-year basis.
_Applicants must meet minimum qualifications at the time of hire._
Preferred Qualifications:
+ 2+ years of experience working in a clinical setting.
+ Experience working with adolescents and/or young adults.
+ Experience with Epic EMR system.
+ Experience working with transgender and gender-diverse youth.
Knowledge, Skills, and Abilities:
+ Critical thinking: Assessing and evaluating individual patients' needs to determine the most suitable solutions based on various factors such as location, insurance, co-morbidity, neuro-divergence, etc.
+ Utilize analytical, evaluative, and interpretive skills to understand and address the needs of individual patients and evaluate adult programs effectively.
+ Ability to apply expertise gained from experience or education to effectively communicate with adult programs and ensure comprehensive information gathering before, during, or after transitions.
+ Ability to establish and maintain effective working relationships with employees at all levels throughout and outside the institution.
+ Outstanding customer service skills.
+ Demonstrated commitment and leadership ability to advance diversity and inclusion.
