UM CASE MANAGER - EMERGENCY - Pomona Valley Hospital Medical Center

Position Summary: The Case Manager is responsible for facilitating the patient’s hospitalization from preadmission to the specific unit admitted. The case manager interfaces with physicians, nurses, social workers and other healthcare team members to expedite medically appropriate cost-effective care. The case manager applies clinical expertise and medical appropriateness criteria to resource utilization and discharge planning. May perform other duties as assigned.

Job Qualifications: [Required] Current unrestricted California RN license. Graduate of accredited School of Nursing with Associates degree Three years acute hospital/clinical experience. One year experience in Utilization Review, Case Management or Quality Management. Demonstrates effective communication skills during interview process. Data entry skills required, or to be acquired during introductory period. Current knowledge/use of Milliman Care Guidelines.

[Preferred Qualifications]:Bachelor’s degree.

Salary range: $61.37 - $90.73
