Unit Care Coordinator (Licensed Practical Nurse/LPN) at Life Care Center of Cleveland

Position Summary:
The LPN Unit Care Coordinator supervises, implements, coordinates, and manages patient care through effective communication with patients, families, and nursing staff, adhering to all applicable laws and Life Care standards.

Education, Experience, and Licensure Requirements:
* Accredited LPN program completion
* Active LPN/LVN license
* Preferred: 1 year geriatric nursing experience
* CPR certification required

Essential Functions:
* Direct daily functions of unit nurses and CNAs
* Manage and coordinate patient care services
* Accurately administer medications
* Respond to emergencies
* Exhibit excellent customer service
* Maintain confidentiality and professionalism
* Collaborate effectively on an interdisciplinary team
