The agency, through its programs and services, encourages the inclusion of people with disabilities into the mainstream of society, including social, recreational, family and work activities.Overview: Veterans Directed Care assist Military Veterans who access Veterans Administrative health care, and have daily care needs, and who transition to community living have services available to them. Tempus Community Services is an approved Area Disability Network Agency (ADNA) and part of the team of services to support that Veteran through their transition to home. Actively participate as a member of Community Services department team and the larger organization in support of business objectives.Essential DutiesConduct Referral, Intake and orientation, and ongoing Case Management services.Reference all VDC rules and regulations to conduct business.Provide input for updates to Tempus VDC CM documents, or processes.Provide video, phone and n person services as required by VDC regulations.Provide monthly reports as required by VCD regulation, to VDC VA Case ManagerWork as part of a team to ensure the best services to Veteran.Establish objective and critical thinking when determining how best to address the Budget Plan of Care that is approved by the VA. Support Veteran to identify needs, and community resources to meet those needs.
