Visiting Nurse - Facilities - Your Health Organization

We are rapidly growing and seeking RN's and LPN's to service our patients throughout the Augusta, GA area. Visiting Nurses will service home and/or Assisted Living Facility patients in care coordination with the provider within their assigned service area(s).

This is a salary-based position. It is a full-time 8-hr position (8:00AM-5:00PM) (Monday-Friday).

The following service area(s) are currently available:

• Augusta, GA

Daily Responsibilities include but are not limited to:

• Assisting providers with any medical needs (i.e., orders, labs, vaccination administration, etc.)

• Traveling to patient homes

• Assessments

• Assessing patients with any acute needs and facilitating a visit with the assigned Provider.

• Skilled Nursing

• Coordinating and Facilitating Telehealth Visits with Providers (including specialty visits)

• Medication Reconciliations

• Chart Documentation

• Chart Audit Reviews/Updates

• Addressing Quality Measures

• Addressing Risk Factors

• Enrolling eligible patients into Chronic Care Management

• Capturing Chronic Care Management

• Strong Communication with assigned Care Team.

• Promptly assisting your assigned Care Team with any medical needs (i.e., administrative duties, orders, vaccine administrations, etc.)

Requirements:

• Active LPN or RN license

• Excellent communication and interpersonal skills with the ability to build strong relationships at all levels of the organization.

• Strong problem-solving abilities

• Ability to work effectively under pressure.
