Visiting Nurse - Registered Nurse - Your Health Organization

We are rapidly growing and seeking RN's to service our facility patients throughout the Savannah area. Visiting Nurses will service Assisted Living Facility and Independent Living Facility patients in care coordination with the provider within their assigned service area(s).

This is a salary-based position. It is a full-time 8-hr position (8:00AM-5:00PM) (Monday-Friday).

The following service area(s) are currently available:

• Savannah area

Daily Responsibilities include but are not limited to:

• Assisting providers with any medical needs (i.e., orders, labs, vaccination administration, etc.)

• Traveling to designated facilities coordinating and Facilitating Telehealth Visits with Providers (including specialty visits)

• Assessments

• Assessing patients with any acute needs and facilitating a visit with the assigned Provider.

• Skilled Nursing

• Medication Reconciliations

• Chart Documentation

• Chart Audit Reviews/Updates

• Addressing Quality Measures

• Addressing Risk Factors

• Enrolling eligible patients into Chronic Care Management

• Capturing Chronic Care Management

• Strong Communication with assigned Care Team.

• Promptly assisting your assigned Care Team with any medical needs (i.e., administrative duties, orders, vaccine administrations, etc.)

Requirements:

• Active RN license

• Experience in a facility setting preferred.

• Excellent communication and interpersonal skills with the ability to build strong relationships at all levels of the organization.

• Strong problem-solving abilities

• Ability to work effectively under pressure.
