Job Details
- Conducting patient interviews and assessments, addressing physical and psychosocial health needs.
- Developing individualized care plans and setting patient outcomes based on assessment.
- Implementing and adjusting care plans to ensure safety and optimize patient outcomes.
- Administering prescribed treatments and medications, documenting patient responses.
- Collaborating with physicians and healthcare teams to coordinate care and treatments.
- Educating patients and families about conditions, medications, and post-hospital care.
- Assisting in discharge planning and ensuring patients receive the appropriate follow-up care.
Skills You Need:
- Current License to practice as a Registered Professional Nurse in New York State.
- Graduate from an accredited School of Nursing; Bachelor’s Degree in Nursing preferred.
- Enrollment in a BSN program within two years and completion within five years of hire.
- Strong clinical judgment and leadership skills.
- Ability to work effectively in a fast-paced, team-oriented environment.
