Job Title: Home Health Admission & OASIS Registered Nurse (RN) - Weekends Only
Compensation: $200 per visit + mileage reimbursement

Position Summary:
Are you looking for your next home care career as a Registered Nurse? Home Health Care Incorporated has a fantastic opportunity for an RN to perform admission and start of care visits around the Twin Cities on weekends. If you have a passion for home care, we encourage you to apply!
Responsibilities:
- Admission/OASIS Visits:Perform Admission / "Start of Care" OASIS Visits throughout an area of the Metro.
- Client Evaluation:Evaluate and assess clients for home health services and create their plan of care.
- Service Recommendation:Recommend services from other disciplines that have not yet been ordered by the physician.
- Collaboration:Collaborate with clients, families, physicians, and other members of the home care team.
Qualifications:
- Licensed as a Registered Nurse in the state of Minnesota.
- Driving is required; must have a reliable vehicle with current insurance and a valid Driver's License.
- Prior Home Health Nursing experience.
- Proficiency with Medicare & OASIS documentation.
- Demonstrated ability to work autonomously.
- Excellent at planning, organizing, and communicating.
