Weinberg ALS Center - Nurse Practitioner - Jefferson Center City (Per Diem) - Thomas Jefferson University & Jefferson Health

The Jefferson Weinberg ALS Center at the Vickie & Jack Farber Institute for Neuroscience seeks an experienced and energetic Nurse Practitioner.

This will be a rewarding opportunity in a leadership position.  The nurse practitioner will work in an outpatient setting as lead referent for the Weinberg ALS Center’s Multidisciplinary clinic and patients’ care. The ALS Weinberg ALS Center’s Clinic provides comprehensive care and state-of-the-art resources to patients with Amyotrophic Lateral Sclerosis and related diseases.  As a key member of the multi-disciplinary team, which also includes research, the successful candidate will provide care to patients including neurological assessment, H&P, outpatient clinic new evaluations and follow up care.  Experience in neuroscience or chronic diseases of the nervous system is a plus but interested candidates with long-term nursing experience are encouraged to apply.

SPECIFIC FUNCTIONS

• Obtains patient history and performs assessment via observation, interview and examination during diagnostic and follow up visits
• Provides direct treatment and management of health conditions within the scope of subspecialty practice via prescriptive authority or referral to other health Centers and Institutions
• Develops, implements and coordinates the plan of care for patients in collaboration with the Clinical Director, the multidisciplinary team of physicians and therapists and the patients
• Coordinates the multidisciplinary team
• May perform diagnostic or therapeutic interventions consistent with certification, law, Jefferson University Physicians privileges
• Provides counseling and education to patients and their families and/or caregivers concerning available community resources, treatment options and disease progression
• Provides telephone follow-up between clinic visits and triage of established patients office (patients’ concerns and questions; prescription renewals; determination of disease progression through ALS functional rating scale; communication of results of studies)
• Communicates appropriate referrals to patients
• Outside of clinic days, works closely with the Center’s dedicated medical and research staff including Speech Pathologist; Dietitian; Genetic Counselor and Clinical Research Coordinators
• Opportunities to participate in Clinical Research and- if interested- assist as evaluator for clinical trials as well as liaison between the research and the clinical team
