Wellness Nurse, LPN/RN - full time (40 Hours) - Lions Gate

Job Description

The Wellness Nurse is responsible for providing residents with first aid, emergency response and health and wellness education.  This role provides PPD, Hepatitis B and Flu shots and Nursing In-services related to general orientation and annual requirements.   

The ideal candidate has a current and unencumbered NJ license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN), certification in CPR/AED/First Aid, and at least 2 years of nursing experience, preferably in a long-term care setting or assisted living community. 

Responsibilities:

• Provides health and wellness education, screenings, and preventative programs for Independent Living residents.
• Provides Independent residents with basic first aid as requested.
• Refers residents to appropriate licensed physicians or other health care providers when indicated by the residents needs through observation.
• Maintains employee health records in compliance with regulations of the New Jersey Department of Health.
• Provides employees with PPDs, Hepatitis B, and Flu Shots.
• Provides/schedules with other department heads or outside sources in-servicing to employees for regulation compliance as well as quality assurance issues as needed.
• Training Administrator for Relias Database for site.
• Tracks on-line employee in-services for all departments; issues reminders prior to deadlines; informs supervisors of employees who are late or lagging.
• Schedules and supervises staff; available to cover staffing when needed.
• Monitors, oversees and administers medications as allowed per state specific guidelines.
• Provides activities of daily living assistance to residents.
• Provides training and orientation to new staff members and ongoing training to current staff.
• Supports staff development personnel with tracking and reporting employee training.
• Performs assessments and re-assessments to determine resident needs and recommend appropriate service plans.
• Assists coordination and completion of admissions, when applicable.
• Presents nursing educational topics during employee new hire orientation.
• Assumes accountability for policies/procedures contained in the employee handbook.
• On rare occasions or staffing shortages, this nurse may have to work as a floor nurse in any unit.

Job Requirements

Licensed as a Licensed Practical Nurse (LPN) Registered Nurse (RN) in the State of New Jersey. 

Certification in CPR(BLS)/AED/First Aid. 

Ability to work occasional weekends.
