Wound Care Nurse (RN or LPN) - Spring Creek Nursing and Rehabilitation Center

We are seeking an experienced and compassionate Wound Care Nurse who is passionate about providing quality care to residents and patients suffering from a variety of ailments. Job responsibilities of the Wound Care Nurse include but is not limited to the following:

Responsibilities:

• Assess patients to determine their risk of infection from wounds.
• Monitor and manage patients who are at risk of developing pressure ulcers.
• Provide information to nursing staff regarding wound care
• Weekly wound rounds with the nurse practitioner
• Manage the wound care process on behalf of the patient.
• Communication with physicians
• Strong knowledge of infection control
• Obtain wound care orders
• Wound treatment changes
• Experience with wound care and wound vacs
• Participates in Performance Improvement activities focusing on patient outcomes, regulatory compliance, risk management, and continuous quality improvement.
• The wound care nurse will report directly to the Director of Nursing
• The wound care nurse will be a part of the management team and participate in the on-call rotation
• Other duties as assigned

*Valid Ohio Nurse License

*Registered Nurse pereferred
