- Provide direct services to assigned individuals in the community
- Deliver Psychiatric Rehabilitative Services (individual and/or groups) in accordance with state regulations
- Offer Skills Training and Development alongside Community Living Supports
- Provide crisis intervention and problem-solving assistance to YES Waiver participants and staff
- Maintain timely and thorough documentation
- Bachelor's degree in psychology, social work, or a related field
- Minimum of 2 years related experience and/or mental health training preferred
- Valid driver's license and vehicle insurance required
- Experience in case management and skills training within the YES Waiver Program
- Experience working with individuals with mental illness and/or addiction issues preferred
- Strong assessment and coordination skills with effective communication abilities
- Knowledge of the social services system and resource connections
- Proficient writing skills for professional communication and documentation
- Ability to remain calm and logical in challenging situations
