Hope For Youth is looking to hire a Clinical Support Specialist for our Youth ACT Program in Bay Shore! To be a successful Clinical Support Specialist, you must be ready willing and able to do the following always:
- Function within the policies and procedures as outlined in program practice guide and employee manual
- Adhere to state regulations and applicable code of conduct
- Work within an interdisciplinary team to assist client in their overall functioning and achieve desired outcomes
- Provide clinical support and Psychoeducation on youth's diagnosis, symptoms and needs.
- Ensure that service delivery is in a trauma informed, culturally and linguistically competent manner
- Engage youth in Psychosocial Rehabilitation and vocational training
- Be available for family for Crisis Intervention and provide support, planning and interventions to stabilize the youth and family.
- Liaise with DSS, schools and other child and youth collateral contacts
- Discharge planning as needed
- Attend trainings and professional development as assigned
- Attend daily team meetings and family meetings as assigned
- Apply agency-identifiedEvidence Based Interventions best suited to meet the client's needs.
- Provide transportation for client to their service location as necessary.
- Provide crisis management and be on a rotation to cover crisis outside normal business hours.
- Perform other resulting and necessary work to meet the needs and requirements of the clients, the program, Hope For Youth, oversight agency and any other regulatory obligations
How you will generally spend your time in this job:
- 60% Direct client therapeutic contact including transportation
- 20% Case documentation, reports, case related meetings
- 20% Staff/team meetings/supervision/training
To qualify, you must have and maintain:
- Bachelor's degree with 2 years' experience working with youth or a master's in social work or related
- Computer proficiency, including Microsoft Office Suite, and experience working with EHRs.
- Valid NYS driver's license
- Physical Requirements: read/write reports and log entries, walking/standing/sitting for prolonged periods, bending/stooping, kneeling/squatting, climbing stairs, reaching overhead, repetitive wrist/finger movements, driving and ongoing daily access to own vehicle
